*This time is reserved specifically for you. You may use as much or as little of it as you would like during your allotted session, however all fees will remain the same.*
IN NETWORK – I accept United Healthcare insurance (and it’s subsidiaries). If you choose to utilize your insurance benefits, it is your responsibility to make sure all insurance benefit information is correct prior to starting services.
OUT OF NETWORK – If you would like to be reimbursed for your visits, please feel free to request a superbill (receipt) on the first of each month. Payment for services is due at the time services are rendered unless other arrangements are made in advance.
**Insurance companies require me to submit a diagnosis for treatment to justify medical necessity. If you have questions about insurance, please reach out to your provider directly.**
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not utilizing insurance benefits an estimate of the bill for medical items and services. This includes costs for diagnostic testing, services rendered, and any other costs associated with your care from this specific provider.
I will give you a Good Faith Estimate (GFE) in writing before your appointment. If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.
